Application Process

Please fill out the online application, print it, and sign the both pages.
Then mail it to:

Please include three years of detailed loss history from your prior workers’ compensation insurance carrier.

NOTE: All policies begin on July 1 and end on June 30.

Once the application has been approved and processed, you will be mailed a quote and payment terms. In order to bind coverage, a signed quote and payment terms (if applicable) must be submitted with a money order or cashier’s check to MOC Insurance Service. COVERAGE IS NOT BOUND UNTIL THE SIGNED ITEMS HAVE BEEN PROCESSED.

Changes in Payroll and Jockey Starts

Your premium is based on the estimated amount of racing stable payroll and number of jockey starts. Adjustments are made on a case by case basis for changes more than 30% and can take some time to be resolved. In the meanwhile, you are responsible for making your installments. Please contact MOC Insurance Services for more information.

Changes in Address

Please notify MOC Insurance Services as soon as you are aware of an address change. MOC will notify the CHSA and the insurance carrier. It is important to have accurate information on every policyholder.
